Cette application est fournie par le groupe Stellantis pour faciliter les démarches administratives liées au retour sur les sites de travail.
Le but est de protéger les collaborateurs, en demandant à chacun de réaliser une auto-surveillance. Pour rappel chaque collaborateur doit présenter une attestation sur l'honneur de la réalisation de ce protocole d'auto-surveillance et de l'absence de symptômes sur 14 jours, pour pouvoir accéder aux établissements du groupe.
Les données saisies dans cette application restent entièrement sous la responsabilité de l'utilisateur, et ne sont pas transmises au groupe Stellantis ou à un tiers. 
Elles sont stockées et accessibles uniquement en local sur le smartphone.
 
L'écran du suivi médical prévu dans l'application ne doit être consulté visuellement que par un médecin ou par un professionnel de santé, par présentation physique du smartphone et de l'application par l'utilisateur. Aucune autre personne n'est habilitée à consulter cet écran.
 
L'écran de l'attestation sur l'honneur doit être présenté physiquement à la personne contrôlant l'accès au site.
 
Nous recommandons à chaque utilisateur de sécuriser l'accès à son téléphone portable par un mot de passe pour éviter que les données de l'application, ou toute autre donnée personnelle, puissent être consultées par d'autres personnes. 
 
Les données de suivi médical sont conservées 14 jours dans l'application, puis effacées.
L'effacement complet des données est possible par désinstallation de l'application sur le téléphone.
 
L'utilisation de cette application est facultative, un formulaire papier étant également disponible.

App of the Stellantis group generating certificates for entry on site
This application is provided by the Stellantis group to facilitate the administrative procedures related to returning to work sites.

The goal is to protect employees, by asking everyone to carry out self-monitoring. As a reminder, each employee must present a certificate on their honor that this self-monitoring protocol has been carried out and the absence of symptoms over 14 days, in order to be able to access the group's establishments.

The data entered in this application remain entirely the responsibility of the user, and are not transmitted to the Stellantis group or to a third party.
They are stored and accessible only locally on the smartphone.

The medical monitoring screen provided for in the application must only be visually consulted by a doctor or a health professional, by physical presentation of the smartphone and the application by the user. No other person is authorized to view this screen.

The sworn statement screen must be physically presented to the person controlling access to the site.

We recommend that each user secure access to their mobile phone with a password to prevent application data, or any other personal data, from being viewed by other people.

The medical follow-up data is kept for 14 days in the application, then deleted.
Complete erasure of data is possible by uninstalling the application on the phone.

The use of this application is optional, a paper form is also available.
